Appointment Requests Sessions are by appointment only, and scheduled times are to be kept in respect of other clients and the therapist. Should a client arrive late for an appointment, that time may be deducted from their session and full price will be charged. Any client arriving 15 minutes or more late for their session will be considered a no show/no call. No Show/No Call No show/no call appointments will be billed via mail. A no show/no call of an initial appointment will result in forfeiture of scheduling further sessions. Cancellations Cancellations are to be made, when possible, 24 hours in advance. It is understood that emergencies arise which require appointment changes. Attempts to honor these requests will be made as scheduling allows. Gift Certificates Certificates are available and must be redeemed within 90 days of purchase. Behavior Inappropriate behavior during a session will not be tolerated and will result in immediate termination of the session. An invoice for full payment will be sent by mail for the scheduled session. Confidentiality Respect and professionalism may be expected at all times. Information obtained during a session is confidential. Exchange of that information requires a signed release by the client. Payment Cash and personal checks are accepted methods of payment. Third party billing my be available with proper consent and verification of benefits. Should you wish to bill your insurance company, a receipt with procedures received will be made available. A $30 fee will be charged for returned checks. Client Information/Guidelines A client information form must be completed and signed prior to the first session and then must be updated yearly. |
